APPLICANT LEVEL 2 FINGERPRINT REQUIREMENTS
FOLLOW THESE STEPS
The State of Florida requires that all individuals looking to obtain a license from the Florida Department of Business and Professional Regulation (DBPR) must complete a live scan Level 2 background check before they can apply for licensing in the following areas: Alcoholic Beverages and Tobacco Athlete Agents Auctioneers Community Association Managers Construction Industry Drugs, Devices and Cosmetics Program Employee Leasing Companies Home Inspectors Hotels, Motels, Apartments and other lodging Mold-Related Services Pari-Mutuel Wagering & Slots Facilities Real Estate Restaurants, Take-outs, Delivery, Caterers and Mobile Food Vendors Talent Agencies
Obtain an ORI number from the Florida Business & Professional Regulation or visit: http://www.myfloridalicense.com/dbpr/servop/testing/documents/finger_faq.pdf for listing of ORI numbers. Bring in your Electronic Fingerprint Form with the ORI number along with your current (not expired) driver’s license, government issued identification card, or passport to WIA Investigation at 125 Basin Street, Ste. 102, Daytona Beach, FL 32114 or call us at (800) 559-2314. Your fingerprints are captured by WIA Investigation and submitted to Florida Department of Law Enforcement (FDLE) for processing. WIA Investigation will provide you with a Transaction Control Number (TCN) report that verifies your live scan submission has been processed. The TCN number can be given to the State Agency that is requesting the background check as it will help them track results.
